  • Duration: 1 year, part-time

The increase incidence of chronic disease presents a considerable challenge to the NHS. This module addresses the issues of pertinent healthcare in the management of a range of chronic diseases and therefore will enable and foster a culture of caring for those patients requiring ongoing support. This course is relevant to a mixture of healthcare professionals working in either primary,  secondary or pre hospital environments and is delivered via a blended learning model of classroom and online learning.

Course Content

The chronic disease course is a delivered via blended learning and only expects professionals to attend 1 day a week for 6 days, the rest will be completed on line. The main elements of the course are:

  • consultation skills
  • health assessments
  • diagnostic tests
  • health screening
  • long term condition monitoring and management
  • Diabetes
  • Chronic kidney Disease
  • Leg Ulcer Management
  • working with individuals who have a variety of health needs

Long Term Conditions is a 20 credit module that is core to the CertHE Chronic Disease. However, the student can then chose two optional modules which include: 

  • Diabetes module (20 credits L6) or
  • Chronic Kidney Disease (20 credits L6) or  
  • Work Based Learning (20 credits L6) or  
  • Leg ulcers (20 credits L6)           .

Workplace support

The student must have the support of their Manager and be permitted to practice under supervision in an appropriate area related to long term condition management.

The module will use a variety of assessments such as written examination, case studies, blogs, competencies and an applied assignment.      

Entry Requirements

  • Registration with the NMC/HCPC and proof of employment in an area clinical areas that deals with long term or chronic conditions.
  • IELTS: 6.0 (with no element below 5.5) is required.

Admissions to the programme are made on a rolling basis, so early applications are encouraged. Applications are made using Bangor’s online application system.

International Students: Because this pathway is part-time and requires a current NMC/HCPC registration and access to a clinical environment, this pathway is not suitable for the international market.

This course is especially designed for healthcare professionals working with long term conditions and will enhance employment and promotion opportunities.

Research / Links with Industry

The academics involved with this CertHE have extensive links with external bodies, companies and the 3rd sector, which are fully utilised in ensuring that the modules are relevant to the modern work and research environment graduates will enter. They are also all experienced Nurse practioners who still have active links to the local NHS trust ensuring that modern clinical practice is embedded within the programme.
