Overview

The course content includes: Global, national and local health care policy related to chronic health conditions and health promotion

NICE guidelines, health care models /pathways/ frameworks for the management of common chronic conditions

Pathophysiology and aetiology of common chronic conditions

Lifestyle factors, wider determinants of health and their impact upon health and well being

The nurse as an educator and promoter of health

Person centred care, shared decision making and co-production

Resilience and empowerment

Motivational interviewing

Self -management of health and illness

The use of technology to support self-management and monitoring of chronic conditions

Complexities of reaching marginalised people e.g. rural communities, socially isolated individuals, those in residential care settings, homeless people, travelling communities

The impact of social isolation on health and well being

Community assets and asset based approaches to promote health and well-being

Care provision across a range of settings including rural areas

Assessment Strategy

-threshold -50% C- to C+ Grade : To achieve a ‘good pass’ on the module, students will be required to demonstrate a good understanding and some critical application of the following: Common chronic conditions and the biopsychosocial impact upon the individual. Differing approaches to care provision across a range of settings in relation to chronic conditions. Lifestyle choices linked to poor health outcomes, related to policy and the role of the nurse as a promoter of health. Tools for empowering individuals to manage their own health. This will need to be supported by appropriate literature.Be focused on the assessment brief, describing the major links between topics with some irrelevant material and/or weaknesses in structure evident. There must be an attempt to present relevant and logical arguments. Assessed material should not contain a large number of factual errors and there should be an attempt to analyse and/or explain problems. Assessed work should be free of major weaknesses in presentation and accuracy. -good -60% B- to B+ Grade: To achieve this grade the student will need to demonstrate a very good understanding and critical application of the following: Common chronic conditions and the biopsychosocial impact upon the individual. Differing approaches to care provision across a range of settings in relation to chronic conditions. Lifestyle choices linked to poor health outcomes, related to policy and the role of the nurse as a promoter of health. Tools for empowering individuals to manage their own health. This will need to be supported by a range of literature. Be focused on the assessment brief, contain logically presented and defended arguments demonstrating an ability to identify, develop and present new links between topics and be free of irrelevant material and/or weaknesses in structure. There must be a very high standard of relevant and logical arguments presented with original interpretation evident. The presentation should not contain factual errors and there should be in depth analysis of a wide range of issues. Be free of weaknesses in presentation and accuracy. -excellent -70> % Students will be required to demonstrate a comprehensive understanding of the facts and principles that underpin the following: Common chronic conditions and the biopsychosocial impact upon the individual. Differing approaches to care provision across a range of settings in relation to chronic conditions. Lifestyle choices linked to poor health outcomes, related to policy and the role of the nurse as a promoter of health. Tools for empowering individuals to manage their own health This will need to be supported by a broad range of appropriate literature. Be focused on the assessment brief, describing the major links between topics with little irrelevant material and/or weaknesses in structure evident. There must be relevant and logical arguments presented with evidence of some original interpretation. The presentation should not contain factual errors and there should be a good analysis of a range of issues evident. Be primarily free of weaknesses in presentation and accuracy.

  • Critically evaluate lifestyle choices linked to poor health outcomes and relate these to evidence and health promotion, evaluating the role of the nurse as a promoter of health and well-being.

  • Demonstrate a critical awareness of the care needs of people with common chronic conditions in a biopsychosocial context.

  • Examine and critically analyse determinants of behaviour change and tools for empowering individuals to improve and manage their own health and well-being

  • Recognise and critically appraise differing approaches to care provision across a range of settings for individuals with chronic conditions.

Choose one case study from a selection and develop a detailed care plan demonstrating critical awareness of the biopsychosocial care needs of the chosen case study. Provide a written rationale for the care plan which recognises and critically appraises differing approaches to care provision across a range of settings.

Create a presentation equivalent to 2000 words which will last for 15 minutes on the following : Lifestyle choices and their impact upon health and well-being with an analysis of behaviour change tools and the role of the nurse in promoting health and well-being linked to evidence. Address learning outcomes: Critically evaluate lifestyle choices linked to poor health outcomes and relate these to evidence and health promotion, evaluating the role of the nurse as a promoter of health and well-being. Examine and critically analyse determinants of behaviour change and tools for empowering individuals to improve and manage their own health and well-being.
