Amcanion cyffredinol
This module will develop the students’ knowledge of the care and treatment of adults with common chronic conditions. The biopsychosocial impact of chronic conditions on an individual will be critically evaluated as will the role of the adult nurse in providing safe structured holistic care through shared decision making and working in partnership supported by best evidence. The skills and knowledge that will be acquired will prepare pre-registration nurses to provide and critically evaluate evidenced- based nursing care and particular emphasis on supporting a co-productive partnership with individuals and their family and promoting self-management strategies.

The course content includes:
NICE guidelines for evidence-based practice in chronic conditions, models /pathways/ frameworks for the management of common chronic conditions; endocrine; auto-immune conditions; respiratory conditions; vascular, stroke and cardiac conditions; neurological chronic conditions; muscular-skeletal conditions.
On-going patient assessment using appropriate monitoring techniques/tools
Patients life style factors and self - management programmes
Global health perspective of chronic conditions and health promotion
Disease management – shared decision-making
Identifying the precursors for ill-health e.g. life style choices, hereditary, culture and social deprivation
Obesity & impact as a chronic condition
Managing acute and chronic pain
On-going patient assessment using appropriate monitoring techniques
Patient life-style factors and self-management programmes to include technology to support independent living
Management of chronic disease in hard to reach populations e.g. veteran, prisoner’s health, homelessness & those living in rural areas
Management of chronic conditions and/or co-morbidities for those with dementia, the ageing population, those with a learning disability or mental health concern
Link between chronic conditions and coping strategies, depression and anxiety
Awareness of common mental health issues e.g. depression/anxiety
Managing social isolation and the correlation between mental health and physical well-being in chronic conditions.

50% C- to C+ Grade :To achieve a ‘good pass’ on the module, students will be required to demonstrate good understanding and some critical application of evidence-based practice in the provision of care of patients who have chronic conditions. This will need to be supported by appropriate literature.

60% B- to B+ Grade: To achieve this grade the student will need to demonstrate a very good understanding and critical application of evidence-based practice in the provision of care of patients with chronic condition/s. This will need to be supported by a broad range of appropriate literature.

Critically evaluate the role of the registered nurse in the care of those with chronic conditions including: holistic structured approaches to the assessment, planning and evaluation of care.

Critically analyse and evaluate evidence based interventions in relation to self-management and co-productive relationships with patients and other appropriate services.

Recognise and critically evaluate presenting clinical features of common chronic conditions and potential acute exacerbations/deteriorations and actions to be taken.

Critically appraise and apply current standards, frameworks, pathways, national and local guidelines relevant to the delivery of care for people with chronic conditions
