The aim of this module is to build upon the experiences gained in the first year and develop a more focussed approach to the application of the key skills required in clinical practice, advancing learning by revisiting key conditions and their management as part of a spiral curriculum.
The students will be expected to become more familiar with the clinical environment and the particular demands and behaviours expected of them in this domain, applying a patient centred approach to their practice. The particular focus in this module will be to facilitate students to perform a competent clinical assessment, be able to derive appropriate diagnoses and set management plans of individuals presenting with a range of conditions as set out in the Physician Associate curriculum, core conditions in category 1A, be able to derive differential diagnoses as in category 2A and competently follow management plans for patients related to 2A and 2B.
Building on the knowledge and experience gained in the first year, this module expands the application of learned skills and critical thinking to develop an understanding of the limits of their competencies and the legal and ethical aspects of clinical practice. Finally, bringing the knowledge and skills together, the subject of holistic patient care will be explored and used as a model for discussion and learning facilitation. More self-directed learning, group discussions and further development of clinical skills related to patient management will facilitate a focussed approach to completing the PA curriculum condition subject areas and prepare students for national examinations and work as a Physician Associate. This subjects covered in this module integrate horizontally and vertically, with the other modules in the programme to enhance the learning experience in the second year.
Students attend placements for the following periods during their second year

Students will undertake procedures and develop competencies as outlined in the Matrix specification of Core Clinical Conditions for the Physician Associate including but not limited to
Perform and interpret a 12 lead ECG. Participate in cardiopulmonary resuscitation to the level expected in Immediate Life Support Training: including oxygen with mask, bag intubation, which medication to use and when, depending upon ECG reading. Undertake respiratory function tests, including the performance of peak flow measurement. Commence and manage nebulised therapy. Commence and manage oxygen therapy. Instruct patients in the use of devices for inhaled medication. Insert a naso-gastric tube (tested in simulation). Undertake nutritional assessment. Undertake appropriate strapping and splinting for common musculoskeletal injuries. Perform fluorescein dye examination of the cornea. Remove loose foreign bodies from under lids. Obtain a cervical smear, cultures for HVS etc. Undertake male and female urinary catheterisation. Perform a urine dipstick test. Undertake simple skin suturing. Be competent in the use of local anaesthetics. Interpret written prescriptions accurately, seeking confirmation when the drug, dose or route of administration are unclear, or where the prescription as written is outside standard practice. Draw up and give intramuscular, subcutaneous, intra-dermal and intravenous injections. Take a venous blood sample, using appropriate tubes for required tests. Obtain an arterial blood gas (ABG) sample. Undertake venous cannulation and set up an infusion and infusion pump. Commence and manage a blood transfusion (in simulation). Measure body temperature. Measure pulse rate. Monitor oxygen saturation transcutaneously. Take nose, throat and skin swabs. Calculate dosage of insulin using a pre-prescribed sliding scale and administer.

Paperwork contained in their portfolios will be a record that they have achieved/performed the competencies/procedures detailed above the order of achievements/performance will be placement dependent

This module is based on the specification of core competences, procedural skills and conditions as identified within the Competence and Curriculum Framework for the Physician Associate.
The module will re-explore conditions explored in the first year and introduce the remaining conditions and competencies, particularly in matrix conditions 1B and 2A and 2B within the context of greater exposure to the clinical environment. The theme of holistic and patient centred care will be developed. This will be achieved by affording learning that is more individualised and reflective, facilitating a more focussed, bespoke learning experience for students allowing completion of studies

An excellent student should have a thorough factual knowledge across all aspects of the module, and be able to detail examples where appropriate, supported by additional reading and knowledge drawn from other modules. The work presents evidence of critical thinking. Especially for the higher A marks, critical analysis and wider reading is essential.

Have a detailed knowledge and understanding of clinical medicine in primary and secondary care as applied to patients across a wide spectrum of ages and have successfully completed all competencies and clinical assessments related to the PA curriculum.

Demonstrate the ability to critically manage both common and less common and more complex conditions, particularly where diagnosis and management decisions have been made by another. Understand the importance of continuity of care, hand-over and requirement of continual assessment and review.

Recognise when the diagnosis of a less common and more complex condition requires input by a supervising doctor or clinical specialist and be able to manage the patient’s condition demonstrating a comprehensive understanding of the principles of treatment as discussed with that clinician.

Demonstrate a deeper understanding of the limits of their competencies and recognise when to refer to colleagues.

Develop an advanced ability to manage patients as a whole and prioritise management steps. Bringing together clinical skills to form overall assessments and judgements on patient management using a holistic approach to care.

Appraise the role of a PA related to the clinical practice of that of a junior doctor but without authorisation to prescribe. Recognise the requirement for continuing professional development and progression to develop new skills and competencies.

Critically request and analyse appropriate therapeutic procedures having considered the evidence base that informs the choice of procedures.
