Overview

This module supports students to develop the knowledge and skills needed to provide person-centred care for people with long-term conditions at all stages of life. Emphasis is placed on understanding the physical, psychological, and social challenges faced by people living with chronic conditions, and exploring strategies to support self-management, shared decision-making, and collaborative care planning.

Students will critically engage with evidence-based approaches to long-term condition management, examining current policy, clinical guidelines, and models of care relevant to general practice. Through an exploration of preventative, curative, and palliative care approaches, learners will deepen their understanding of delivering inclusive, culturally sensitive care within integrated health and social care systems. This module prepares students to use their specialist practice knowledge to improve health outcomes and reduce health inequalities in their clinical roles, especially in General Practice.

Assessment Grading Criteria The following grading thresholds outline the expectations for academic performance and professional capability in relation to the role of the Specialist Practitioner. These criteria are aligned with the module learning outcomes, the standards set by the Nursing and Midwifery Council (NMC), and the academic regulations of the University, which expect students to demonstrate progression in critical thinking, academic integrity, and reflective, evidence-informed practice. Assessment outcomes are benchmarked against university-wide marking criteria to ensure consistency, fairness, and academic rigour.

Excellent Pass (A-) The student produces work of high academic quality, critically evaluating evidence-based practices with depth and originality to co-produce person-centred care plans. They demonstrate advanced insight into integrating preventative, curative, and palliative approaches with ethically grounded, collaborative decision-making across all ages . The role of integrated care systems is expertly critiqued, with innovative, well-evidenced patient education and self-management solutions designed to improve outcomes .

Good Pass (B-) The student presents a competent, well-structured analysis of evidence-based approaches to long-term condition management, clearly applying person-centred and collaborative care planning . They demonstrate sound understanding of care approaches across the lifespan and begin to integrate shared decision-making meaningfully . Multidisciplinary roles and care systems are clearly assessed, with relevant and feasible strategies for patient education and self-management.

Threshold (Pass – C-) The student demonstrates a foundational ability to evaluate evidence and apply principles of person-centred care . They show a basic understanding of preventative, curative, and palliative care approaches, including limited but appropriate use of shared decision-making . There is an introductory awareness of the roles of multidisciplinary teams and early attempts to develop patient education and self-management strategies .

• Apply leadership, clinical judgement, and specialist knowledge to deliver inclusive, culturally sensitive, and effective care that responds to the complex needs of individuals, families, and communities within general practice.

• Critically analyse preventative, curative, and palliative care approaches, incorporating shared decision-making strategies to support individuals in managing their health and wellbeing.

• Critically assess the role of multidisciplinary care teams and integrated care systems in general practice, and design patient education and self-management programmes that improve health outcomes and address health inequalities.

• Critically evaluate evidence-based approaches to managing long-term conditions and apply person-centred care principles to develop individualized care plans in collaboration with patients, families, and multidisciplinary teams.
